  3. The cost of the procedure is $65.00 dollars, which you must pay by Money Order.

The general requirements for both adults and minors are:

  • Recent Birth Certificate issued by RENAP with the CUI number or the DPI in original plus a photocopy of both sides.
  • A Money Order for $65.00 dollars to pay for the procedure.
